从25年前出台起搏器治疗指南开始到现在,ACC/AHA相继推出了几十种实践治疗指南。来自美国北卡罗来纳州的心血管研究和治疗中心主任Sidney C. Smith在TCT TV上向人们讲述了这些指南诞生背后的故事。
ACC/AHA手下有17个指南委员会,每个委员会中有16~20名工作人员,一个新指南的出台,意味着要占用每位工作人员时间是3~4年,这主要取决于指南的类型。一些别的团体如血管造影学会和介入协会也会参与相关指南的出台。
Smith说:“过去,每个协会之间1年进行一次商谈,但是,现在每2天就会召开一次紧急会议。所有到会的人员都必须到场,并且在工作完成之前,都必须关掉手机和邮件。”
处理争端
最近几年,委员会成员在很大程度上也受各种利益冲突的制约,为了杜绝这一现象,所有成员都必须公开自己所持有的股份、收受的好处,是否是某一临床试验的带头人等信息,除了经济上,我们要避免的更是学术上的纠纷。虽然说委员会成员可以参与指南制定或者跟所进行的试验相关的工作,但是他们不会是项目主要负责人。
几年以前,ACC/AHA计划成立最新一个部门,专门收集最新信息,以修改现行指南,要求部门所有成员都不受各种利益冲突的制约,但是最后这种愿望终未实现,原因是符合条件的专家数量非常有限,因此,现在ACC/AHA只能在减少偏倚的基础上对专家进行筛选。
指南应用中面临的问题
当问到说为何现实中许多的心脏学家并未遵循指南时,Smith指出,原因有两个:一是在新指南出现之前,现行指南是用来给大家提供参考的标准;二,新出现的指南都太长或者太详细,以至于医生在还没有完全掌握的情况下或者在患者情况危急的情况下,你不能说让患者等待你把一本360页的STEMI指南读完。
“协会也正采取各种方法将指南进行缩减,使其被充分利用,可以考虑把它做成小册子,那就可以放在口袋,随用随看。”Smith解释说,“在不久的将来,随着对卫生保健制度的加强,可能对于指南的应用情况也会增加。学会也在考虑出台有关肥厚性心肌病和瓣膜病的相关指南,而PCI和STEMI的最新指南在明年初应该会跟大家见面。”他同时还鼓励所有医务人员提供反馈信息,因为第一个PCI指南的出台,就是协会成员从1,600份**搜集出来的,“我非常欢迎任何一位关心或者对此感兴趣的人给我们提出建议,因为我们也致力于将每个指南更贴近我们现实以更好地发挥实际应用价值。”Smith说。

Since joining forces 25 years ago to develop guidelines for the use of pacemakers, the American College of Cardiology and American Heart Association have jointly published dozens of evidence-based practice guidelines.
In an interview with TCT TV, Sidney C. Smith, Jr., MD, director of the Center for Cardiovascular Science and Medicine at the University of North Carolina, Chapel Hill, gave a behind-the-scenes look at the guideline process, which he helps oversee.
Together, the AHA and ACC currently sponsor 17 guideline committees, each with 16 to 20 members who serve for three to four years. Depending on the topic, other groups such as the Society for Cardiovascular Angiography and Interventions sometimes co-sponsor the guidelines.
In the past, a committee would meet for just a day, Smith said. Now, the committees get together for two days of intensive review. "We want people who will really be there and turn off [their] phones and e-mail and get the job done," Smith said.
Handling conflicts
Conflict of interest rules have been tightened for committee members in recent years. Members must disclose stock ownership, honoraria, and whether they serve as principle investigator on any studies.
"Someone might have an NIH grant that’s affected by the guidelines," Smith said. "There are intellectual conflicts we need to be aware of, not just monetary [conflicts]." Principle investigators can serve on committes working on guidelines related to the trials they lead; however, they cannot chair them.
A few years ago, the AHA and ACC decided to organize a focus update committee – which revises existing guidelines – without sitting any members who had any potential conflicts of interest. Smith said the experiment was a failure because there was not enough subject-area expertise on the committee.
Now, the AHA and ACC are still learning how to "move away from bias but still have expertise" on the committees, he said.
Problems with guideline adherence
When addressing why more cardiologists do not follow guidelines, Smith pointed to two possible reasons.
First, he said, the AHA and ACC could do a better job of communicating when new guidelines appear. Second, the guidelines often are too long and detailed for physicians to absorb while patients wait. "You can’t leaf through 360 pages on STEMI while a patient is in the ER," he said.
The committees are now working on condensing some guidelines.
"The guidelines I’ve seen used most often are on pacemaker placement – those you can carry around in your pocket," Smith said.
In the near future, increasing use of performance measures by health care systems will probably drive higher adoption of guidelines.
The committees are now considering guidelines on hypertrophic cardiomyopathy and valvular heart disease. Updates for PCI and STEMI should arrive early next year, said Smith.
He encouraged all practitioners to provide feedback. For the first PCI guidelines, the committee sifted through some 1,600 comments. "I would encourage anyone with interest or concern to let us know. We’re interested in making guidelines more useful," he said.
