【ESC2009】抗凝治疗取得新进展

ESC2009 于8月29日在西班牙的巴塞罗那开幕，两天来发布了多个印像颇深的临床研究，其中在抗凝抗栓方面的进展最为引人注目。
RELY研究：对比观察抗凝新药直接凝血酶抑制剂Dabigatran与经典抗凝药华发林对房颤患者血栓栓塞事件的疗效以及安全性，研究入选了18113例患者，平均CHADS评分为2.1-2.2之间（0-1分、2分、3分各占三分之一），分为三组：华发林组、Dabigatran 110mg 2次/日组和Dabigatran 150mg 2次/日组。主要结论：与华发林组相比，Dabigatran 150mg组降低卒中和周围栓塞事件34%，出血事件两者相当；Dabigatran 110mg组疗效与华发林相当，但出血事件明显减少。ESC专家述评表示，这是近50年抗凝治疗取得的重大进展，优点是不需像华发林一样监测INR值，治疗窗口宽，还可以根据患者发生栓塞风险和出血风险选择Dabigatran 剂量。Dabigatran的上市无疑将改变目前抗凝治疗的现状。
